Here is what you do:

1. Collect 48 Total Points (Point Box, UPC Box, or Both)
2. Complete the “Get the Gear Form” (button below).
3. The form will be sent to your email.
4. Print ALL PAGES of the email confirmation.
5. Place [Point Box, UPC Box or Both] + [email confirmation] into an envelope and mail to:

We will send you an EVOL Tshirt in approximately 6-8 weeks after receiving your completed package. EVOL reserves the right to change out our “gear” based on what we currently have in stock. Tshirts are adult unisex style. Thanks!
