Note: We have made changes to our program.

The old program was “24 points”
The new program is “48 points”, but that is because we doubled the point value of everything
(Example: Bowls used to be 1.5pts, now Bowls are 3pts).
We have also changed our packaging.

The old packaging used the “UPC Box”
The new packaging uses the “Point Box”

You can combine old packaging (UPC Box) and new packaging (Point Box) to redeem your gear.

Here is what you do:

1. Collect 48 Total Points (UPC Box, Point Box, or Both)

UPC Box Point system:
Pizzas = 4 pts
Flatbreads = 3 pts
Bowls = 3 pts
Classic Burritos = 2 pts
Deli Wrap Burritos = 2 pts
Premium Burritos/Wraps = 2 pts
3oz Mini Burritos = 1 pt

2. Complete the form below.
3. The form will be sent to your email.
4. Print ALL PAGES of the email confirmation.
5. Place [UPC Box, Point Box or Both] + [email confirmation] into an envelope.

We will send you an EVOL Tshirt in approximately 6-8 weeks after receiving your completed package. EVOL reserves the right to change out our “gear” based on what we currently have in stock. Although we ask for Tshirt style, we will usually fulfill with unisex Tshirts. Thanks!
